Certification of Church Membership
To the Pastor* or Designated Church Staff Member:
Every year we ask our students to verify their current church membership. This form should be completed by the Pastor or appropriate church staff member who oversees church membership to show good standing as a member of the church. This verification is crucial as it helps us ensure accurate communication with the Southern Baptist Convention regarding student membership and funding. It is required of all students, regardless of the churches denominational affiliation.
While information regarding your state and local affiliations may not directly affect your student, it will provide us with key information regarding the churches we serve, it is very helpful to us.

Annual Deadline: December 31st.
